EQuIP for Social and Community Services

ACHS accredits healthcare organisations in a variety of ambulatory care settings, including Social and Community Care. ACHS Ambulatory Care Accreditation will help providers deliver consistent, high reliable care in critical areas impacting the quality and safety of individuals in your care and staff.  

The Evaluation and Quality Improvement Program (EQuIP) is designed to provide a framework which will support social and community services to ensure the provision of safe, high-quality care and services, and to achieve continuous quality improvement.

EQuIP is based upon principles that support best practice, facilitate a culture of quality, and guide improvements to care and outcomes. These principles are designed to be applied to all aspects of service within a healthcare organisation, including Patient Focus, Effective Leadership, Continuous Improvement, Evidence of Outcomes and Best Practice.

Social care and community care refer to services and support provided to individuals in their homes or community settings, rather than in hospitals or other institutional settings. These services may include personal care, such as bathing and dressing, as well as assistance with activities of daily living, such as meal preparation and housekeeping. Community care may also involve access to healthcare services, such as physical therapy or chronic disease management, and social support programs, such as counselling or transportation services.
