Originating from the globally recognised Hospital Survey on Patient Safety Culture (HSOPS 2.0) developed by the Agency for Healthcare Research and Quality (AHRQ), the PSCS has been adapted for the Australian context by the Australian Commission on Safety and Quality in Health Care (ACSQHC).
