QI In Action: Patient-Centred Care in Radiotherapy

Island Hospital, a leading quaternary healthcare provider based in Penang, Malaysia, has long been at the forefront of integrating advanced technologies into patient care. Established in 1996, the hospital has more than 80 full-time specialists across a broad range of medical and surgical specialties. It is recognised as a top healthcare destination, especially for international patients.  

In 2024, Island Hospital was honoured with the ACHS International Quality Improvement (QI) Award in the Clinical Excellence and Patient Safety category for its submission, Tattooless Radiotherapy: Advancing Patient-Centred Care. This groundbreaking initiative demonstrates how cutting-edge technology can be harnessed to enhance clinical outcomes while prioritising patient comfort and dignity.  


Advancing Radiotherapy with SGRT Technology  

Traditionally, patients undergoing radiotherapy are marked with small, permanent tattoos to assist in accurate positioning during treatment. While clinically effective, these visible marks can cause discomfort, emotional distress, and lasting cosmetic effects.  

The primary aim of this project was to introduce a paradigm shift in radiotherapy by replacing tattoo-based alignment methods with Surface Guided Radiation Therapy (SGRT) enabled tattooless radiotherapy. This approach aligns strongly with modern patient-centred care principles and was designed to improve comfort, reduce emotional distress, and support better cosmetic outcomes. Using a proprietary system that includes high-precision 3D camera arrays and time-of-flight technology, the SGRT solution generates real-time 3D imaging of the patient's surface. This allows clinicians to precisely align and monitor patients throughout treatment without the need for permanent skin markings.  

The hospital’s SGRT system comprises three integrated camera sets calibrated to the radiographic isocenter, ensuring accurate patient positioning. These systems are rigorously tested and maintained through a robust quality assurance program.  


Patient-Centred Outcomes  

By eliminating the need for tattoos, Island Hospital has significantly improved the treatment experience for its patients:  

  • Preservation of skin integrity and natural appearance  

  • Reduced emotional burden from visible treatment marks  

  • Improved patient comfort throughout the radiotherapy process  
     

Island Hospital’s transition to tattooless radiotherapy not only reflects its commitment to clinical excellence but also showcases its patient-first philosophy. The integration of SGRT has allowed the hospital to offer a less invasive, more compassionate radiotherapy experience without compromising safety.
